What To Bring
• Health insurance card and your spouse's, if applicable; all insurance forms; and paperwork such as pre-authorizations
• Valid driver's license, state ID card or passport
• Advance Directive documents if you have them
• All current medications in their original containers
• Outside lab test results and pertinent x-rays not taken at this hospital
• Personal hygiene items
• Clothing for your hospital stay and discharge
• Eyeglasses, hearing aid
• Reading, writing materials, other personal items
• If desired, cash for newspapers, gift shop and other items. Less than $20 is recommended.
• Credit card number and expiration date (not the credit card itself)
ICU/CCU patients should limit personal items. Bring only absolutely necessary items, such as robe, eyeglasses, hearing aid or other prosthetics. We provide slippers and grooming items. if you forget any of these items, please ask your nurse, or have family members bring the item from home.

Good Samaritan Medical Center cannot take responsibility for loss of these items. Should items of value be brought, we recommend they be deposited in the safe in Admitting. Ask Admitting or your nurse about safe deposits. Be sure to ask for a receipt, or send items home with a family member.

During other hours, please register in the Emergency Department.
Consent For Treatment
You will be asked to sign a consent-for-treatment form giving us permission to treat you.
Advance Directives are written instructions you make regarding your treatment should you be unable to speak for yourself. The Durable Power of Attorney for healthcare allows you to designate someone to speak for you if you are unable to make decisions about your care. On admission, adult patients are asked if they have a current Advanced Directive. If so, the Directive is placed in the patient's medical record. If not, an informational pamphlet and form are available. You may request these from Admitting via email or by calling (561) 650-6391. You are not required to have an Advance Directive to receive care.
